Q. What caused my dogs chronic kidney failure, and how is it treated?

Leslie Sinclair, DVMLeslie Sinclair, DVM says: Chronic kidney failure can occur in dogs of any age, but the likelihood increases with age. Over time, kidneys begin to lose their function and shrink in size. The kidneys compensate for decreasing function until they have very little ability to work, and then they begin to fail. Kidney transplantation is available for cats and has been explored for use in dogs, but the cost and complications associated with transplant rejection, as well as the ethical considerations associated with identifying a donor dog and extending the life of an elderly dog, make it an unrealistic option for now.

The most common symptoms of kidney failure are increased urination and increased thirst, known in medical terms as polyuria and polydypsia, or PU/PD. Once the condition advances to the point that the kidneys are no longer able to filter ammonia (a by-product of the breakdown of protein in the diet) from the bloodstream, the dog develops uremia, a toxic condition characterized by nausea and vomiting. A veterinarian analyzes a blood sample to further assess the degree of kidney failure. The most widely used indicators are the blood urea nitrogen (BUN) and creatinine values. Both of these are products of digestion that build up in the bloodstream when the kidneys are functioning poorly. The degree to which the BUN and creatinine values are increased correlates with the level of kidney failure.

Kidney damage cannot be reversed, but there are supportive measures that allow a dog with kidney failure to live comfortably, sometimes for years after the condition is first diagnosed. Dogs suffering from kidney failure are often dehydrated when diagnosed, and intravenous fluids are used to reverse the dehydration and reduce the amount of ammonia-like products in the dogs bloodstream. Antinausea medications can reduce vomiting and discomfort caused by uremia. Many dogs with kidney failure are anemic (lacking an adequate number of red blood cells) because they cannot produce a sufficient amount of the hormone erythropoietin, which is made by the kidneys and regulates the production of red blood cells by the bone marrow. Some dogs become so anemic that they require a blood transfusion or medication to stimulate their body to produce more red blood cells.

Because the blood circulating in a dogs body must go through the kidneys, the narrowed pathways found in damaged kidneys may cause a dog to have high blood pressure. This aspect of kidney failure has not yet been well studied because blood pressure measurement is difficult in dogs. As better equipment becomes available for monitoring dogs blood pressure, improved recognition and treatment will be available.

Once a dogs condition has stabilized, the veterinarian recommends a diet low in protein to prevent symptoms from reoccurring. If your dog develops chronic kidney failure, closely monitor him at all times, and work with your veterinarian to provide your dog with the best possible quality of life despite this irreversible condition.

Dear Monica -- my baby girl and my boy died the same way. Fluid entered their chest and they drool and I had to put them to sleep. I write this to ask you to try to become vegetarian as cows, chickens and pigs are like your baby ... they are animals with mom and dad. This message may be out of line but I only wish the best for you.

My 5 yr Pomeranian was attacked by kidney failure in a 4 week time frame. She was hospitalized for 4 days getting IV fluids & received frozen plasma. She showed some hopeful signs. I brought her home to follow-up w/ a holistic vet. She was maintaining a stable, yet high BUN & creatinine level. She started getting IV fluids at the vet daily.Rechecked her levels & they were holding. We backed off the IV fluids over a weekend to see how she would do. She started vomiting once an hour it seemed, over the weekend. Started giving her anti-nausea meds. They helped some, however she still wouldn’t eat. Monday, got her back to the vets to start back on aggressive IV fluids. Her little veins were almost blown. She would not eat, I had to force feel her food & Pedialite for several days prior & after the IV fluids started. This little girl had a huge appetite prior to all of this happening. Within 48hrs, she became weak, a lifeless pup and her BUN & creatinine levels were up. Her little head just wobbled like a doll. She had no energy to even move. I tried EVERYTHING I could, yet I felt like I was running against a time clock to pull her through. I was with her all hours of the day and night nursing her to health. This past Tuesday morning, she was scheduled to go back to the VET for another round of IV fluids. I knew that something more was wrong. Her breathing had changed during the night; she had a crackling sound in her chest & massive drool coming from her mouth. She wouldn’t even try to swallow. My vet confirmed my fears. My baby girl was fading away fast. Fluid had entered into her lungs. She was fighting to hang on for me. She would follow me with her eyes. I looked her in her eyes that morning. With tears in mine, I told her it was OK to go. Go have fun & play in heaven. I love you so much. She had tear roll out from her eye as if she knew what I was saying. I NEVER gave up on her. I tried everything I had available to me. She had hard time breathing. I asked our vet to help in letting her go in peace. I held her for 2 hours after she passed. Just loving on her & telling her how sorry I am this horrible disease won. I feel like I let her down. My family has a huge void. She was my baby & little best friend. 4 weeks ago, she was happy and playing. Today she is an angel. I pray more research is done to help find more treatments. It‘s believed her kidneys got toxins from popular chicken jerk treat made in China. Within 6 months, she had the treats & 3 days of Metacam. She had a good past medical history. I am waiting 4 the call when I can come & pick her ashes up. I want her home, with family to be in peace. I was worried that her soul & spirit wouldn’t pass over & was looking for me or didn’t know where to go. I visualize Jesus holding her. She had her tongue hanging out & wagging her tail in HIS arms. There were many animals all around them. It was such a peaceful feeling. I will always be her Mommy. R.I.P. Sweet one. Prissy - 10/6/06 ~ 3/6/12
